MAJOR: fd: Get rid of the fd cache. Now that the architecture was changed so that attempts to receive/send data always come from the upper layers, instead of them only trying to do so when the lower layer let them know they could try, we can finally get rid of the fd cache. We don't really need it anymore, and removing it gives us a small performance boost.
