coverity: fix MISRA violations

Fixes for the following MISRA violations:
- Missing explicit parentheses on sub-expression
- An identifier or macro name beginning with an
  underscore, shall not be declared
- Type mismatch in BL1 SMC handlers and tspd_main.c
